COVID Vaccine Booster Verification
For those who serve in a ministry that aids in Sunday worship services
In order to keep all service participants and in-person congregants safe, the Vestry and Staff of St. Paul’s Church are asking all persons who serve in a ministry that aids in worship services on Sunday mornings to show proof of your vaccine booster by January 31st.
This includes:
† Acolytes/Crucifers
† Adult Choir members
† Eucharistic Ministers
† Lectors
We are also asking all CYF Ministry volunteers, Godly Play teachers, and clergy and staff to do the same.
Please scan or take a picture of your vaccine card and upload it to the form below. All uploads are secure and are only viewable (not downloadable) by staff and ministry leaders.
If you do not submit an image of your vaccine card by January 31st, and it is determined by staff that you have not been fully vaccinated and boosted against COVID-19, you may be asked to suspend your participation in said ministry. You may not return until it is determined by state, local, and diocesan guidelines that it is safe for all that you return.
